THIS SERVICE AGREEMENT (this “Agreement”) is made and entered into by and between THE PET STATION COUNTRY CLUB, LLC, a Limited Liability Company in KENTUCKY, and the person identified as the “Owner” below. This Agreement applies to all visits by the Owner’s dog or cat to The Pet Station Country Club. The Pet Station Country Club provides dog and cat boarding, Dog Daycare (Daycation), Training, Grooming, and Day Boarding services. Owner hereby engages The Pet Station Country Club to provide the services for Owner’s pet(s) as set forth on the website, the information sheet provided to the Owner, the terms and conditions set forth in this Agreement, and in any supplemental pet information or reservation forms provided in connection with this Agreement by The Pet Station Country Club.

By clicking ‘I Agree’, ‘Accept’, or any similar button below, and/or by accessing or using any services from The Pet Station Country Club, you (the Owner) acknowledge that you have read, understood, and agree to be bound by this Service Agreement in its entirety. If you do not agree, do not proceed or use our services.

Payment

Owner agrees to pay all fees for services rendered, veterinary care, or other services owed on the day of pick up. Prices are subject to change without notice and seasonal rates may apply.

Fees  

Owner is responsible for bagging Owner’s pet’s food into individual serving bags prior to dropping the pet off for boarding. Owner acknowledges and understands that a $0.55 per meal charge will apply if the pet’s food is not in individual serving bags.  

Boarding check-out and check-in times apply during the week (Monday-Friday) and do NOT apply on the weekends (Saturday and Sunday). Check-out throughout the week is before noon. A $5 per pet per hour late charge will be applied to the final bill if the pet is not picked up by noon. Boarding check-in throughout the week (Monday-Friday) is after 1 pm. A $13 per pet early check-in fee will apply if the pet is checked in prior to 1 pm. Check-in and check-out on the weekends (Saturday, Sunday) can take place anytime during business hours (Saturday: 8 am – 3 pm; Sunday: 9 am – 1 pm).

Cancellation, Deposits, Credits

If the Owner needs to cancel a boarding reservation, The Pet Station Country Club requires at least three (3) days’ notice prior to the arrival date during non-peak times. Peak times require at least thirty (30) days’ notice prior to the arrival date for cancellation (see front desk for peak dates). Cancelling outside the windows listed results in a 100% refund or credit to the Owner’s wallet account. Cancelling within the windows listed results in forfeiture of 100% of the reservation deposit amount. The Pet Station Country Club will only hold deposits and/or credits on accounts for a period of one year, after which time the credit will be removed from the account and no longer be available for use with any service.  

Daycation reservations are required and The Pet Station Country Club cannot accept walk-ins. Dogs scheduled for Daycation on a particular day that do not cancel 24 hours in advance will be charged the daily rate or package visit.

Health and Vaccinations

Owner represents to The Pet Station Country Club that, to Owner’s knowledge, the pet obtaining services has not been exposed to any contagious diseases within a thirty-day period prior to check-in.  

The Pet Station Country Club reserves the right to refuse to accept a pet at check-in for any reason, including without limitation, if it appears to The Pet Station Country Club that the dog or cat is sick, injured, in pain, or that its behavior could jeopardize the health or safety of other dogs or cats, or staff.  

Owner acknowledges that the pet is current on all required vaccinations, including flea treatment and a negative fecal float, as well as the recommended Canine Influenza vaccine, and such other vaccinations as required by The Pet Station Country Club. Owner agrees to provide documents confirming such vaccinations as requested by The Pet Station Country Club, and shall provide proof of vaccinations prior to any entry into The Pet Station Country Club facilities. Owner understands that Capstar administration is mandatory for all pets (unless otherwise documented by a veterinarian) and agrees to a $4.50 charge per pet on each boarding stay.  

Owner understands that all pets at The Pet Station Country Club are fully vaccinated. However, it is still possible for any pet to become ill, even if vaccinated. Owner understands that this is not due to any circumstance or condition at The Pet Station Country Club and agrees not to hold The Pet Station Country Club liable in the event of illness or death during or after the pet’s stay.  

If at any time the dog or cat is found to have fleas or ticks, The Pet Station Country Club may provide the appropriate flea or tick removal treatment, and Owner authorizes The Pet Station Country Club to provide such service at an additional expense to Owner.

Emergency Medical Treatment 

Owner acknowledges that, in the unlikely event the boarded pet becomes ill or injured, or if the dog or cat has a pre-existing condition which is aggravated by its stay, and requires professional attention, The Pet Station Country Club will attempt to notify the Owner or Emergency Contact at the phone numbers provided. The Pet Station Country Club, at its sole discretion, may engage the services of a 24-hour emergency veterinary clinic or a veterinarian of its choice and/or administer medicine or give other necessary attention to the dog or cat (First-Aid, CPR, or other life-saving methods), and Owner authorizes The Pet Station Country Club to provide any such service at an additional expense to Owner. In cases The Pet Station Country Club believes to be critical, it may take the boarded pet to the veterinarian first before trying to contact the Owner. If medical treatment is refused by the Owner, The Pet Station Country Club, at its sole discretion, may engage the services of a veterinarian and/or administer medicine to make the dog or cat as comfortable as possible until picked up by the Owner or Emergency Contact, and Owner authorizes The Pet Station Country Club to provide any such service at an additional expense to Owner. If The Pet Station Country Club cannot reach the Owner or Emergency Contact, it will make health care decisions for the dog or cat based on the recommendations of available professionals.  In the unfortunate event that the boarded pet passes away at The Pet Station Country Club, arrangements have been made with the local Veterinary Hospital to keep the pet at their facility until the Owner returns. If the Owner does not wish to be notified while away or prefers other arrangements, the Owner must inform The Pet Station Country Club in writing.

Emergency or Natural Disaster

In an emergency or natural disaster, every effort will be made to contact the Owner or Emergency Contact to retrieve the boarded pet. Owner agrees that The Pet Station Country Club, at its sole discretion, is authorized to transport and/or make temporary alternative arrangements to house and care for the boarded pet until the Owner or Emergency Contact can retrieve the dog or cat. Owner understands that it may not always be possible to safely evacuate the dog or cat.

Group Activity 

Owner understands that while the dog is staying at The Pet Station Country Club, it will come into contact with other dogs. Every effort will be made to ensure the safety of all guests. Owner acknowledges that, in the unlikely event the Owner’s dog is injured by another dog, Owner releases The Pet Station Country Club from any liability for such injury.  

If the Owner’s dog injures another dog, Owner will be solely responsible for any injury to the other dog(s) as well as Owner’s own dog, and Owner releases The Pet Station Country Club from any liability for such injury.

Dog Behavior  

Owner agrees that if the dog becomes aggressive, destructive, or disruptive, or acts in any manner that puts The Pet Station Country Club, its representatives, or employees in danger, The Pet Station Country Club will attempt to notify the Owner or Emergency Contact to make other arrangements for the dog. Owner acknowledges that dogs that are kenneled together are from the same household. Owner assumes all responsibility for any aggressive behavior, fighting, or injury that one dog may inflict upon another while they are together.  The Pet Station Country Club reserves the right to separate any dogs who are boarding together at its discretion for the dogs’ own safety. If the Owner’s dogs are not compatible boarding in the same room unit, Owner will be billed and responsible for the cost of more than one room.

Photos and Video Release 

Owner agrees to allow The Pet Station Country Club, its representatives, and employees to use the dog or cat’s name and any images or likeness of the dog or cat taken while the dog or cat is staying at The Pet Station Country Club, in any form or format, for use at any time, in any media, marketing, advertising, illustration, trade, or promotional materials.

Personal Property 

Owner agrees that The Pet Station Country Club, its representatives, and employees will not be liable or responsible for any lost, stolen, or damaged personal property belonging either to the Owner or the dog or cat. Owner also understands and agrees that the dog’s collar may be removed in the kennel and play areas to prevent injury to the dog.

Abandonment 

Owner understands and agrees that if the pet is not picked up by the scheduled pick-up date and time, late fees will continue to accrue daily until the pet is retrieved. If the pet is not picked up within fifteen (15) calendar days after the scheduled pick-up date, and despite reasonable efforts by The Pet Station Country Club to contact the Owner and Emergency Contact (including phone calls, emails, and/or certified mail to the addresses and numbers provided), the Owner fails to retrieve the pet or make satisfactory arrangements for its care, the pet may be deemed abandoned.  

In such event, The Pet Station Country Club reserves the right, at its sole discretion and in compliance with applicable Kentucky law:  

(a) to continue boarding the pet at the Owner’s expense;  

(b) to transfer the pet to a local animal shelter, humane society, or animal control authority; and/or  

(c) to take any other action deemed necessary for the welfare of the pet.  

Owner agrees to remain fully responsible for all boarding fees, late fees, veterinary expenses, transfer costs, and any other charges incurred until the pet is retrieved or otherwise resolved. Owner further agrees to reimburse The Pet Station Country Club for any reasonable attorney’s fees and costs incurred in enforcing this provision or addressing the abandoned pet.

Owner agrees that Owner has read this entire Agreement, has had the opportunity to discuss it with The Pet Station Country Club to Owner’s satisfaction, and agrees to its terms.

Grooming Addendum (if applicable)  

Owner understands that there are instances where grooming can expose a hidden medical problem or aggravate a current one, along with a chance of injury, stress, and/or trauma.  

Owner understands that senior pets have a greater chance of injury, stress, and/or trauma during grooming.  

Owner understands that tangled or matted pets have a greater chance of injury, stress, and/or trauma as well as nicks, clipper irritation, and mental or physical stress, etc.  

Owner understands that there is an additional fee for matted pets, because removing mattes is an extremely difficult and time-consuming procedure that interferes with the entire grooming schedule for all clients and their pets that day; furthermore, it is both physically and mentally taxing on the pet as well as the pet stylist.  

Owner understands that it is possible that the only option for matted pets is to clipper them down to the length necessary to remove the mattes, which can mean in some cases that the coat is taken down to a very short length.  

Owner understands that The Pet Station Country Club cannot accommodate the pet if it is aggressive or extremely hyperactive, due to the many dangers and chances of injury involved when a pet is unsettled during the grooming procedures.  

Owner understands that there is an additional fee for aggressive or extremely hyperactive pets if the grooming is able to be completed.

Group Play Addendum (if applicable) 

Owner understands that The Pet Station Country Club (or the “Company”) has relied upon Owner’s representation that the pet is in good health and has not injured or shown aggression or threatening behavior to any person or dog in admitting the dog for services at the facility.  

Owner further understands that The Pet Station Country Club, its owners, staff, partners, and volunteers will not be liable, financially or otherwise, for injuries to the dog, the Owner, or any property of the Owner while the dog is participating in services provided by The Pet Station Country Club.  

Owner understands and agrees that any problems with the dog, behavioral, medical, or otherwise, will be treated as deemed best by staff of The Pet Station Country Club in their sole discretion, and in what they view as the best interest of the animal. Owner understands that Owner assumes full financial responsibility and all liability for any and all expenses involved in regard to the behavior and the health of the dog.  

Owner further understands that there are risks and benefits associated with group socialization of dogs. Owner agrees that the benefits outweigh the risks and that Owner accepts the risk. Owner desires a socialized environment for the dog while attending services provided by The Pet Station Country Club. While in the care of The Pet Station Country Club, Owner understands that while the socialization and play is closely and carefully monitored by staff to prevent injury, it is still possible that during the course of normal play the dog may receive minor nicks and scratches from roughhousing with other dogs. Any noticeable injuries to the dog will be pointed out by staff upon pick-up.

Owner further acknowledges that, in the unlikely event the dog becomes ill or injured, or if the dog has a pre-existing condition which is aggravated by its stay, and requires professional attention, The Pet Station Country Club will attempt to notify the Owner or Emergency Contact at the phone numbers provided. The Pet Station Country Club, at its sole discretion, may engage the services of a 24-hour emergency veterinary clinic or a veterinarian of its choice and/or administer medicine or give other necessary attention to the dog, and Owner authorizes The Pet Station Country Club to provide any such service at Owner’s additional expense. In cases The Pet Station Country Club believes to be critical, it may take the dog to the veterinarian first before trying to contact the Owner. If the Owner refuses medical treatment for the dog, The Pet Station Country Club, at its sole discretion, may engage the services of a veterinarian and/or administer medicine to make the dog as comfortable as possible until picked up by the Owner or Emergency Contact, and Owner authorizes The Pet Station Country Club to provide any such service at Owner’s additional expense. If The Pet Station Country Club cannot reach the Owner or Emergency Contact, it will make health care decisions for the dog based on the recommendations of available professionals.  

Owner understands that by allowing the dog to participate in services offered by The Pet Station Country Club, Owner hereby agrees to allow The Pet Station Country Club to take photographs or use images of the pet in print, electronic form, or otherwise for publication and/or promotion.  

Owner further understands that Owner is solely responsible, financially or otherwise, for any harm or damage caused by the dog while the dog is attending any services provided by The Pet Station Country Club.  

Owner understands that if the dog is not picked up on time or by the date specified in a separate agreement, Owner hereby allows The Pet Station Country Club to take whatever action is deemed necessary for the continuing care of the dog. Owner will pay The Pet Station Country Club the cost of any such continuing care upon demand by The Pet Station Country Club. Owner understands that if Owner does not pick up the animal, The Pet Station Country Club will proceed per the guidelines provided by Kentucky Statute for animal abandonment. Owner also acknowledges that Owner will be fully responsible for all attorney’s fees and associated costs if Owner abandons the dog.
